Our specialized team of hospice professionals is available 24 hours a day and consists of the attending or hospice physician, nurses, home health aides, social workers, therapists, volunteers, counselors, chaplains and bereavement coordinators. They work with the patient, family members, and attending or hospice physician to develop a plan of care that will meet the patient's and family's needs. We focus on helping each patient live his or her life to the FULLEST!
