Building Famillies, Inc. Mental Health Targeted Case Management Services

Targeted Case Management is a service available to personally help individuals available by assessing their needs for services and communtiy resources including but not limited to the following:

-Assistance with Daily Living

-Affordable Housing

-Educational Enrichment

-Tutoring

-Vocational Taining Programs

-Mentoring Programs

-Support Groups

-Training and Education

-Adult & Youth Support Services

-Assistance in applying for Social Security and Public Assistance Programs/Benefits

Who is eligible?

Program participants must be Medicaid eligible recipients who are currently enrolled in the Medicaid program. An individual is generally interested in, or referred to targeted case management services because the adult is looking for help with coping with the stress and emotional challenges of life, or children who are experiencing behavioral issues in school or at home. All clients must undergo an assessment and consent to participate in the program with monitored progress by the Targeted Case Manager assigned to them as their support.

What is Targeted Case Mangement Serivces

Targeted Case Management Services are available for children and adults who are Medicaid recipients experienceing emotional difficulties. Our primary goal is to provide and link services fpr pur clients, advocate on their behalf, coordinate and monitor services that will assist them in improving thier mental health. Case Managers are certified through the Agency for Health Care Administration and are knowleadgeable about resources for clients.

Targeted Case Managers link clients to services and monitor their progress.

Targeted Case Management Services are provided in the client's home, school, community and/or at the site of the services needed based on the developed Individualized Service Plan.

Targeted Case Managers work to empower the cleint by enhancing access to community resources and ensuring that the client is ableto utilize these resources.
