Community Health Mission is a private, nonprofit 501(3) tax-exempt medical organization that provides free access to primary health care to adults aged 18-64 years old, who live or work in Chatham County and surrounding areas and who are uninsured (including no Medicare and Medicaid). Community Health Mission was formed from the merger of two organizations, Community HealthCare Center and Savannah Health Mission in 2006. Community Healthcare Center and Savannah Health Mission have provided essential primary healthcare and preventive health services to the uninsured in the Savannah area. The merger brought the strengths of the respective organizations to fully and efficiently serve the ever-increasing uninsured population. The new merged organization Community Health Mission (CHM) operates 5 days a week with one night clinic and convenient hours for our working patients. All services provided to the uninsured are at no cost to the patient. Community Health Mission has a long history of working with other organizations in the community. During the past 5 years CHM has collaborated with St. Mary's Health Center, MedBank, and Community Cardiovascular Council. Because of this collaboration, the four organizations have been able to leverage funding to continue their services, improve the quality of care rendered to their patients, and coordinate services to better serve the uninsured. Currently, CHM is housed in a single-story building, leased to CHM for $1.00 a year from Memorial Health University Health Center. CHM is accessible to persons with physical disabilities, including individuals with mobility difficulties, visual and hearing impairments. The CHM provides designated handicapped parking and is accessible to the mobility impaired by either ground or ramp. The building has a total square footage of 2,829 square feet. There are four major function areas the current location. They are: clinical areas, administrative areas, space for MedBank area, and support area.
