Heart City Health Center is a non-profit, Federally Qualified Health Center (FQHC) providing affordable, high quality, health and dental care to Elkhart County residents. We offer a sliding fee scale based on income and family size for the Uninsured and also accept Medicaid, Medicare and some private insurances.

It’s important to have a doctor who knows your specific health history and understands your individual needs throughout every stage of your life. That’s why we want to be your medical home for everything from prenatal and pediatric care to adult and geriatric care.

This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233 (g)-(n).
